Resumo: Introdução: Anormalidades tireoidianas têm sido associadas com obesidade e muitos mecanismos vêm sendo propostos para explicar tal associação. A obesidade vem sendo associada com aumento de volume tireoidiano, aumento dos níveis séricos de hormônio tireoestimulante (TSH), piora da resistência insulínica (RI), com consequente aumento de fator de crescimento semelhante à insulina-1 (IGF-1) e, ainda, alterações nas secreções de adipocitocinas/citocinas, sendo potenciais fatores de desenvolvimento e progressão do câncer de tireoide. Não há recomendação para screening de nódulos tireoidianos, por meio de ultrassonografia (US), em indivíduos com obesidade. Objetivo: O presente estudo visa identificar a frequência de alterações ultrassonográficas tireoidianas em pacientes com obesidade correlacionando os achados com níveis de fator de crescimento semelhante à insulina-1 (IGF-1), proteína C reativa (PCR) e parâmetros clínicolaboratoriais de RI. Materiais e Métodos: Estudo transversal que incluiu 67 pacientes com obesidade graus II e III e 44 controles com IMC normal, de 18 a 60 anos. Todos os participantes foram submetidos a uma anamnese, exame físico, ultrassonografia de tireoide e análise laboratorial. Resultados: Pacientes obesos apresentaram níveis mais elevados de homeostatic model assessment - insulin resistance (HOMA-IR) e PCR (P <0,001). A distribuição dos achados da tireoide, com probabilidade limítrofe ((P = 0,054), evidenciou que o grupo controle apresentou com maior frequência tireoide normal (54,5%), e o grupo obesidade padrão heterogêneo (38,8%). Não houve diferenças estatisticamente significantes no padrão da tireoide entre os pacientes com ou sem RI (P = 0,129). Pacientes obesos e pacientes com RI apresentaram volume estatisticamente maior da tireoide que os controles (P <0,001). Dezessete (25,4%) pacientes do grupo obesidade e 11 (25%) pacientes do grupo controle apresentaram pelo menos um nódulo na tireoide com indicação de PAAF em 6 (35,3%) e 1 (9,1%), respectivamente. IGF-1 alterado baixo foi mais frequente entre os pacientes com resistência insulínica (14,5%; P = 0,052). O nível de PCR foi estatisticamente mais elevado no grupo de padrão heterogêneo do que entre os indivíduos com tireoide normal (P < 0,05). Conclusões: Esse estudo não observou uma correlação positiva entre as variáveis resistência insulínica e níveis de IGF-1 com a presença de nódulos ou outras alterações tireoidianas, porém os achados tireoidianos de pacientes com obesidade evidenciaram um maior volume da tireoide neste grupo, com padrão heterogêneo da glândula mais evidente. O grupo obesidade apresentou níveis mais elevados de resistência insulínica e PCR, o que pode estar diretamente envolvido com os achados encontrados. Alguns pacientes apresentaram níveis de IGF-1 mais baixos, principalmente a categoria com RI. PCR foi o único biomarcador que teve correlação com achado de heterogeneidade.
Abstract: Introduction: Thyroid abnormalities have been associated with obesity and many mechanisms have been proposed to explain this association. Obesity has been associated with increased thyroid volume, increased thyroid stimulating hormone (TSH), worsening of insulin resistance (IR), with a consequent increase in insulin-1- like growth factor (IGF-1), and still changes in secretions of adipocytokines / cytokines, being potential factors for the development and progression of thyroid cancer. There is no recommendation for the screening of thyroid nodules by ultrasonography (US) in obese individuals. Objective: The present study aims to identify the frequency of thyroid ultrasonographic changes in obese patients correlating findings with insulin-1-like growth factor (IGF-1), C-reactive protein (PCR) and clinical-laboratory IR. Materials and Methods: A cross-sectional study that included 67 patients with class II and III obesity and 44 controls with normal BMI, from 18 to 60 years. All participants were submitted to an anamnesis, physical examination, thyroid ultrasonography and laboratory analysis. Results: Obese patients had higher levels of homeostatic model assessment - insulin resistance (HOMA-IR) and PCR (P <0.001). The distribution of thyroid findings, with borderline probability (P = 0.054), showed that the control group had a higher normal thyroid frequency (54.5%) and obesity group had the heterogeneous standard (38.8%). There were no statistically significant differences in the thyroid pattern among patients with or without IR (P = 0.129). Obese patients and patients with IR presented statistically higher thyroid volume than controls (P <0.001). Seventeen (25.4%) patients in the obesity group and 11 (25%) patients in the control group had at least one nodule in the thyroid, with indication of FNA in 6 (35.3%) and 1 (9.1%), respectively. Low IGF-1 was more frequent among patients with IR (14.5%; P = 0.052).The level of PCR was statistically higher in the heterogeneous pattern than in those with normal thyroid (P <0.05). Conclusions: This study did not observe a positive correlation between the variables insulin resistance and IGF-1 levels with the presence of nodules or other thyroid changes, but the thyroid findings of patients with obesity showed a greater thyroid volume in this group, with a heterogeneous pattern of the gland more evident. The obesity group had higher levels of insulin resistance and PCR, which may be directly involved with the findings. Some patients had lower IGF-1 levels, mainly the category with IR. PCR was the only biomarker that correlated with heterogeneous findings.
