dc.description.resumoIntrodução: A mama é um importante símbolo de feminilidade e sexualidade. A presença de hipertrofia e de afecções neste órgão gera efeitos de ordem psicosocial. A Hipertrofia mamária, além de causar problemas de ordem física e psicológica, pode em função do seu volume dificultar a detecção de alterações nos exames de imagem (ultra-sonografia e mamografia). Objetivos: Avaliar a sensibilidade e a especificidade dos exames de mamografia e de ultra-sonografia na detecção de alterações mamárias em portadoras de hipertrofia mamária em seus diversos graus. Material e métodos: Estudo prospectivo realizado em 60 portadoras de hipertrofia mamária no serviço de cirurgia plástica do Hospital Universitário da Universidade Federal do Maranhão (HUUFMA), no período de abril de 2005 a outubro de 2006. Foi realizada uma correlação entre exames de imagem com o exame histopatotógico, padrão ouro, das 120 peças cirúrgicas obtidas com a cirurgia plástica de redução mamária. Resultados: A faixa etária estudada foi de 16 a 72 anos e as mamas com peso de até 800g corresponderam a 65% de todos os casos. Nos exames de imagem, o BIRADS 1 foi o mais freqüente (65,8 na ultra-sonografia e 26,7% na mamografia). No histopatológico foi diagnosticado 47,5% de lesões benignas não neoplásicas, sendo 7,5% com risco relativo levemente aumentado. Conclusões: Na ultra-sonografia, a especificidade não apresentou variações com a literatura. Enquanto que a sensibilidade da ultra-sonografia apresentou-se comprometida com a hipertrofia. Na mamografia a especificidade foi baixa na população estudada e a sensibilidade, independente da faixa etária, apresentou resultados compatíveis com mamas densas. Para população estudada, a mamografia foi deficiente para detecção das alterações diagnosticadas no exame histopatológico.por
dc.description.abstractIntroduction: The breast is an important female and sexually symbol. The presence of hypertrophy and affections in this organ generate effects of psychosocial order. The breast hypertrophy ,even though cause problems of physical and psychological orders, may difficult detection of alterations in the image exams (ultrasonography and mammography) because of its volume. Objectives: To evaluate sensibility and specificity of mammography and ultrasonography on the detection of breast alterations in patients with breast hypertrophy in many degrees. Material and Methods: Prospective study realized in 60 patients with breast hypertrophy in the plastic surgery service from Federal University of Maranhão Hospital, in the period of april 2005 to october 2006. It was made a correlation between the image exams with hystopathological exams, golden standard, of the 120 surgery pieces obtained with the breast reduction plastic surgery. Results: The group age studied was 16 to 72 years and the breasts that weighted until 800g corresponded 65% of all the cases.On the images exams, BI-RADS 1was the most frequent (65,8% on the ultrasonography and 26,7% on the mammography).About the histopathology was diagnosticated 47,5% of non benign neoplasias lesions with 7,5% of a discrete increase on its relative risk. Conclusions: On the ultrasonography, specificity didn´t presented variations according to the literature. Although ultrasonography sensibility presented compromised the hypertrophy. The specificity of mammography was low on the population studied and the sensibility, independent of the age, presented compatible results with dense breasts. To the population studied, mammography was deficient to detect alterations diagnosed on the histopathology.eng
